Who Should Attend
Individuals employed with a county alcohol and drug abuse authority who are not on any State or Office of the Inspector General Medicaid Exclusion List
REGISTRATION IS LIMITED TO 40 PARTICIPANTS.
Prerequisite
Participants must:
Hold a bachelor’s degree from an accredited college or university or be licensed as a registered nurse through the S.C. Department of Labor, Licensing and Regulation
Have one year of experience working with the target population.
Be familiar with the resources in their communities.
Description
This training in Medicaid Targeted Case Management (MTCM) is approved by the S.C. Department of Health and Human Services. MTCM is a means for achieving beneficiary wellness through communication, education, and service(s) identification and referral. MTCM is a time-limited process that provides an organized, structured process for moving beneficiaries through the process of change and toward the goal of self-sufficiency. This is a collaborative process between the case manager and the beneficiary that helps them gain access to the resources they need to maintain their well-being. This training will teach you how to perform the primary case management functions of assessment, planning, identification of resources, referral, and monitoring.
