Who Should Attend
Individuals employed with a county alcohol and drug abuse authority who are not on any State or Office of the Inspector General Medicaid Exclusion List
REGISTRATION IS LIMITED TO 40 PARTICIPANTS.
Prerequisite
Participants must:
• Hold a bachelor's degree from an accredited college or university or be licensed as a registered nurse through the S.C. Department of Labor, Licensing and Regulation.
• Have one year of experience working with the target population.
• Be familiar with the resources in their communities.
Description
This training in Medicaid Targeted Case Management (MTCM) is approved by the S.C. Department of Health and Human Services. MTCM is a means for achieving beneficiary wellness through communication, education, and service(s) identification and referral. MTCM is a time-limited process that provides an organized, structured process for moving beneficiaries through the process of change and toward the goal of self-sufficiency. This is a collaborative process between the case manager and the beneficiary that helps them gain access to the resources they need to maintain their well-being. This training will teach you how to perform the primary case management functions of assessment, planning, identification of resources, referral, and monitoring.
