Job Description: Coordinates the full continuum of care within the agency's MAT program.
Job Responsibilities:
• Provides a full spectrum of treatment services to clients referred from within the agency and other referral sources.
• Using a therapeutic background, works with individuals and families to help resolve conflict and develop new coping skills.
• Works with a variety of clients age 18 and older.
• Provides intake, assessment, case management, crisis management, individualized treatment plans, clinical placement, and treatment of clients, to include individual, family, and group therapy.
• Coordinates care with and staffs cases within the MAT program with the onsite provider (OVP).
• Performs such duties as on-call, outreach counseling, drug screens, and community liaison as scheduled.
• Develops monthly, quarterly, and special reports as needed for DAODAS MAT deliverables.
• Performs related duties as outlined by immediate supervisor.
• Complies with SUMTER BHS/DHEC/DAODAS/CARF policies.
Required Qualifications:
• Master's degree in the human services field.
• Ability to be certified as an Addictions Counselor by Addiction Professionals of South Carolina within three years of hiring date.
• Good written and oral communication skills.
• Knowledge of referral services.
• Ability to coordinate a program consisting of 75+ clients (but will not be the rendering clinician to all of the MAT clients).
Preferred Qualifications:
Salary: $40,000 - $52,000
